CLAUSE:

In connection with the implementation of the requirements of Regulation of the European Parliament and of the Council (EU) 2016/679 of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46 / EC (general data protection regulation) ("RODO"), the Director of the Statistical Office in Warszawa informs about the rules and your rights related to the processing of your personal data.

I. Administrator's indication: The administrator of your personal data is the Director of the Statistical Office in Warszawa having its registered office in Warszawa (02-134 Warszawa) at 1 Sierpnia 21 - hereinafter "Administrator".

II. Objectives and legal basis for the processing of your personal data: Your personal data will be processed on the basis of Art. 6 item 1 point e) the RODO as necessary to perform a task carried out in the public interest or in the exercise of public authority entrusted to the administrator;
Your personal data will be used to carry out tasks under the Law of 29 June 1995 on Official Statistics (Journal of Laws of 2016 item 1068, with later amendments).

III. Obligation to provide personal data: Processing your personal data for the purpose of completing an order for statistical data, answering a submitted question and implementing other information services, including subscription to the newsletter, is voluntary.

IV. Information about recipients of your personal data: Your personal data will not be transferred to other recipients.

V. Periods of personal data processing: Your personal data will be stored until the legal basis for processing them from the date of submission of the inquiry, order, application, other information services, including newsletter subscription, or until you withdraw or limit your consent to the processing of data.

VI. Profiling and automated decision-making: The data provided by you will not be profiled nor will it be the basis for an automated decision.

VII. The rights of the data subject: the right of access to personal data, including the right to obtain a copy of this data; the right to request correction of personal data - if the data is incorrect or incomplete; the right to request the deletion of personal data; the right to request a restriction to the processing of personal data; the right to transfer personal data; the right to object to the processing of personal data, including profiling; the right to withdraw consent to the processing of personal data, the right to lodge a complaint to the supervisory body of the President of the Office for Personal Data Protection.

VIII. Forwarding of personal data to entities outside the European Economic Area ("EEA") or international organizations: Your personal data will not be shared with entities established outside the EEA and international organizations.
